Below are links to the application materials for the West Virginia Commission on the Arts grants. Some of the materials are in Adobe Acrobat format. The free Acrobat Reader for Windows or Macintosh is available here; it allows you to read the materials online in your browser, via a plug-in, or to save the materials for offline viewing. It will also permit you to print out the materials as needed. To read in the browser, just click one of the links below; to save to your hard drive and read offline, right-click the link (Windows) or Option-click (Mac) and choose Save. The latest version of Reader is 8; at least version 6 is required. If you don't know your version, start Reader, then go to Help, click About Acrobat Reader; the version will be in the box that pops up.
Big plus! The forms online can also be filled out in the Acrobat Reader, then printed and mailed in to the Commission office. No need for a typewriter. However, the free Reader software will not let you save the forms with your information filled in.
Suggestion: save the blank forms to your computer. Print out the forms first and make your notes on a draft, then open the forms and fill them out in the computer. If you don't have a copier, you might choose to print two copies (one for file), since you cannot save the forms with the information filled in.
When you fill out the form using your browser, use the mouse to click in the first area to fill in. The Tab key will take you from field to field. You can back up and change your input by using the mouse. Remember, you can't save the form with your information filled in!

To be approved for a grant processed through the West Virginia Division of Culture and History, an individual or organization must have a current Vendor Registration with the State of West Virginia. If not already registered, a registration form (WV1A) is available from the Division. When completing the form, the most common commodity code used by applicants has been code 962: Miscellaneous Services. The two most common mistakes made by applicants when completing the form have been: (1) the failure to update the registration form with a change of address; (2) and, the failure to name a bank in item 11. To register as a vendor or to update vendor registration, complete the form and mail an original copy signed in blue ink directly to: West Virginia Division of Culture and History, Arts Section, The Cultural Center, 1900 Kanawha Boulevard, East, Charleston, WV, 25305-0300.
Individuals and organizations applying for grants processed through the West Virginia Division of Culture and History must also have a certified Taxpayer Identification Number. Internal Revenue Service Form W-9 is used to certify taxpayer identification using an individual applicant's Social Security Number or a corporation's Federal Employer Identification Number. The Division requires an updated Form W-9 for each grant submitted. Form W-9 is available from the Division or from the IRS web site. Mail the original copy of Form W-9 signed in blue ink to the Division at the above address.
